Who is APM Communities?
APM Communities delivers NDIS Local Area Coordination services as a NDIS Partner in the Community across parts of Western Australia, Queensland and the Northern Territory.
Our Local Area Coordinators help people:
- Understand the NDIS
- Check if they may be eligible
- Apply for NDIS access
- Understand and use their NDIS plan
- Connect with supports and services in their community
- Prepare for plan reviews and changes in circumstances
We also help build more inclusive and accessible communities through local partnerships and community projects.

Getting started with the NDIS
If you, or someone you care for, needs support accessing the NDIS, APM Communities can guide you through the process.
Our LAC’s will help you apply to the NDIS and gather the right information and evidence.
The NDIA will contact you to confirm your eligibly for the scheme. Once your plan is finalised and approved, your LAC will help you put your plan into action.
If you believe you meet the eligibility criteria for the NDIS and would like help to apply for access contact APM Communities.
Eligibility
You may be eligible for the NDIS if you:
- Are aged between 0 to 65
- Are an Australian Citizen, a permanent resident, or hold a special category visa
- Have a permanent disability that has a significant impact on your daily life
Not eligible for the NDIS?
Not every person with a disability will be eligible for an NDIS plan.
If you or the person you care for need help, we can still support you with information ad help to connect you to local mainstream and community services in your local area.
