Aged care services from APM

My Aged Care Assessments
If you're finding some daily tasks more difficult, a My Aged Care assessment can help identify the support you need to continue living independently at home.
Our assessors explain the process clearly and take the time to understand your strengths, goals and daily routines.
You can also have a family member, friend or support person with you during the assessment.

AN-ACC Assessments
For residential aged care, AN-ACC assessments help determine the level of care funding needed to support each resident.
These assessments are completed by experienced accredited health professionals on behalf of the Australian Government.
