Welcome to APM Assessment Services

We’re here to support you through your aged care assessment and help you understand what happens next.

APM Assessment Services works with the Australian Government to deliver Aged Care Needs Assessments for older people.

We work with you, and with your carers or family if you choose, to understand your needs and goals.

This helps you access services that can support your independence wherever possible and help you maintain a good quality of life.

After your assessment

After your assessment, APM Assessment Services will provide written information to you, and to your registered supporter if you have one.

This information explains any formal decisions made under the Aged Care Act 2024 about your My Aged Care services.

You may receive one or more of the following notifications.

Notice of Decision

What this is: A formal letter sent after your assessment.

It confirms the decision about your aged care, including whether you are eligible for services and which services you have been approved for.

It also explains how you can ask for the decision to be reviewed if you do not agree with it.

Your assessment outcome and support plan

What this is: A written summary of your assessment.

It outlines your needs, goals, and the aged care services you have been approved to access.

Do you have a registered supporter?

If you have chosen a registered supporter (such as a family member, carer, or trusted person) during your registration, we will share copies of your post‑assessment documents with them.

This may include your Notice of Decision and your support plan.
